Association Between Prostatic Volume, Metabolic Syndrome and Left Ventricular Mass: A Cross Sectional Study

Abstract: Ageing in men is associated with increased prevalence of cardiovascular risk factors and benign prostatic hyperplasia (BPH) with both entities possibly representing downstream manifestations of a common pathogenesis. There is paucity of data on the association of BPH and left ventricular hypertrophy (LVH). This study evaluated relationship between prostate volume, common cardiovascular risk factors and LVH. It was a cross sectional, prospective, hospital-based study. Thirty patients with benign prostatic enlargement (BPE) and 30 age- matched male controls without BPE were studied. All the subjects had clinical, biochemical, prostate ultrasound and echocardiographic evaluation done. SPSS IBM 20 was used to analyze data. Mean age was 65.60 ± 8.11 years (range: 54-82years). Subjects with BPE compared to those without did not significantly differ in age (66.50± 7.75 vs. 64.70±8.49; p=0.39). Significantly higher percentage of subjects with BPE had abnormally low HDL-cholesterol and high blood pressure compared with subjects without BPE. Eight (26.7%) of subjects with BPE in contrast to none of the subjects without BPE had metabolic syndrome (Chi2 = 9.231; p=0.002). Left ventricular mass index were significantly higher in subjects with BPE than in those without. None of the subjects without BPE as compared with 7 (23.3%) of subjects with BPE had echocardiographic determined LVH. Echocardiographic indices that significantly correlated with prostatic volume were: LVPWd (r=0.326, p= 0.011), IVSTd (r= 0.267, p=0.039), LVMI(r=0.308, p= 0.017), LAD (r= 0.494, p <0.0001) and AOD (r= 0.352, p= 0.006). The conclusion was that BPE is associated with increased left ventricular mass index and metabolic syndrome, mostly driven by elevated blood pressure and low serum HDL-cholesterol.

Abstract: Bone marrow metastases as the initial presentation of a solid tumour are uncommon, especially if the primary neoplasm site cannot be identified during the diagnosis presentation despite laboratory testing and imaging techniques. In these situations, bone marrow examination can lead to a final diagnosis of the primary malignancy. We report an unusual case of osteoblastic metastases from an unknown primary site – this was because no relevant lesions were found from either imaging or endoscopic examination. The patient was sent for a bone marrow biopsy, revealing a morphology replaced by glandular structures and an immunohistochemistry consistent with a gastrointestinal origin. Bone marrow examination was the clue to the final diagnosis and led to specific treatment.

Abstract: To investigate the correlation between retinol binding protein 4 (RBP4), lipoprotein associated phospholipase A2 (LP-PLA2), we studied the clinical phenotype of coronary heart disease (CHD) and the severity of coronary artery disease. Our studies showed that the changes of retinol binding protein 4, LP-PLA2 serum concentration has a relation with the severity of coronary artery disease progression, but no clear relationship with the degree of coronary artery lesion and count. Furthermore, LP-PLA2 concentration increases with the increase of the severity of coronary artery disease and disease occurrences.

Abstract: Intravenous leiomyomatosis (IVL) is a benign smooth muscle tumor which may cause fatal outcome. Furthermore, the clinical manifestations are variable and nonspecific, typically resulting in misdiagnosis. Computerized Tomography (CT) played a vital role in the diagnostic process and pre-surgical assessment. Here we describe one case of IVL extending from the inferior vena cava to the right atrium and ventricle, then review the literature and discuss the imaging findings and clinical manifestation of this tumor. It is suggestive for clinicians to be aware of this condition and highly suspect a middle-aged woman with a history of uterine leiomyoma presenting with an inferior vena cava filling defect or mass shadow to early diagnose and treat.

Abstract: Langerhans cell histiocytosis (LCH) is a rare disorder characterized by abnormal proliferation of cells with a Langerhans cell phenotype, which comprises a wide range of clinical presentations. The reported patient is a 55-year old female with multy-sistem LCH and pulmonary tuberculosis. During diagnostic processing, infiltrates of Langerhans cells (S-100+, CD1a+) in the epidermis, intestinal mucosa and bone marrow, were discovered. Transbronchial needle biopsy of the lung revealed non-specific clusters of macrophages (CD68+, CD1a-). Although CD1a was negative, positive CD68, thoracic MSCT and clinical findings indicated pulmonary LCH. Corticosteroid treatment led to significant clinical improvement. Seven months after treatment with corticosteroids the patient developed a spontaneous left-sided pneumothorax. Thoracic HR (high resolution)-MSCT was highly suggestive for active Mycobacterium tuberculosis (MT) infection together with the positive QUANTIFERON-TB test and typical symptoms. One week after starting standard triple antituberculotic therapy the patient developed hypovolemic shock. Despite all medical efforts, the patient died on the thirteenth day of intensive treatment for multiple organ failure.

Abstract: Objective: This paper aims to explore the trend, extent and impacting factors of the change in the psychological status of women before and after hysterectomy. Methods: A prospective study was performed to investigate the psychological statuses of 98 female subjects who underwent hysterectomy upon admission and three months after the hysterectomy. Results: The self-rating of the psychological status survey showed significantly lower factor scores including the postoperative BDI and SCL-90 (except paranoid factors) scores than those before the surgery and the same HAMD score as the nurse-administered rating scale. The incidence rate of moderate to severe depression dropped from 48.98% to 13.54% after the surgery. Differently aged patients showed different change extents of SCL-90, and the postoperative BDI and HAMD scores were significantly positively correlated with the preoperative levels. The preoperative HAMD score was negatively correlated with the length of education and positively correlated with the chief complaint. The postoperative HAMD score was positively correlated with the parity and the presence of medical complications and negatively correlated with age. Conclusion: Hysterectomy benefits the psychiatric and somatic health of the patients, and the women who were younger, more prolific, less educated, complain of dysmenorrhea or menorrhagia and have other medical disorders are more prone to perioperative depression.
